About the Client

The client is an innovative healthcare organization specialized in chronic care infrastructure providers, helping healthcare systems manage and coordinate specialized medical care for patients. By bringing together multidisciplinary care teams, healthcare providers, and community resources, the organization enables a more coordinated, patient-centered care. The client is building a connected healthcare ecosystem designed to support both patients and care providers.

Business Challenges

1. Fragmented Healthcare Data

Clinical, claims, eligibility, and operational data were spread across multiple legacy systems. So, teams lacked unified, trusted views for healthcare data analytics and decision-making.

2. Strained Data & BI Environment

The existing Azure SQL EDW and Power BI environment struggled with growing data volumes and reporting demands. Performance and scalability limitations made it harder to support evolving business needs.

3. No Governed Environment for Risk Modeling

Risk scoring and population health analytics models lacked a centralized, governed environment for execution. The organization needed to bring data, Python, and advanced analytics together on a secure platform.

4. Complex Partner Data Exchange

Sharing data with external partners securely and efficiently was challenging. The existing approach couldn’t get near-real-time collaboration and governed access.

5. High Infrastructure Overhead

Managing legacy infrastructure and multiple disconnected workloads added operational complexity and cost. The organization needed a modern platform that could simplify operations while supporting future growth.

Solution Implemented

Built a Centralized Healthcare Data Platform: Our team migrated key EDW (Enterprise Data Warehouse) workloads to Snowflake, creating a centralized and governed foundation for healthcare data analytics. This helped bring critical data domains together on a scalable platform.

Ensured a Validated Data Migration: Clinical, claims, and eligibility data were migrated with validation and reconciliation processes. This helped maintain data consistency and confidence in the information used for downstream analytics.

Enabled In-Platform ML & Python: The new environment enabled RAF (Risk Adjustment Factor) scoring and risk models to run natively within Snowflake. This brought advanced healthcare data analytics and provided a governed execution layer.

Simplified Secure Partner Sharing: Snowflake Reader Accounts enables secure and governed data sharing with external partners. This created a more streamlined approach to collaboration without relying on traditional data movement.

Modernized Reporting Workloads: BI workloads were offloaded to snowflake data platform to reduce pressure on the existing production environment. This provided a more scalable foundation for reporting and analytics as business demands continued to grow.

Key Results Achieved

Unified Healthcare Data: Clinical, claims, eligibility, and partner data brought together on one governed platform.

Scalable EDW, BI & Analytics: Offloaded EDW and BI workloads reduced pressure on legacy systems and created a more scalable analytics environment.

AI-Driven Risk Modeling: Native ML and Python enabled in-platform RAF scoring and population-health modeling closer to trusted data.

Secure, Near-Real-Time Data Exchange: Governed data sharing and secure ingestion simplified partner collaboration and near-real-time integration.

Lower Infrastructure & Operational Overhead: Zero-copy cloning and native platform capabilities reduce data duplication, infrastructure effort, and operational complexity.
